Faculty and Advisors (Web) Access Form
Notice:
OSU Policy 2-0701 requires all individuals who have access
to the Student Information System (SIS) to complete the
University-sponsored FERPA training before access is granted.
If you have not already done so, please go to the Registrar's
Office website and complete the FERPA tutorial.
Click here
to download the "Access Request" form once you
have successfully completed the FERPA training.
